I have read the information about Kingfisher Camp and give permission for the person named above to attend, take part in all activities, and declare that s/he is fit to do so.
I accept that there may be short periods when my son/daughter will not be under direct supervision; these periods will however be kept to a minimum.
I give permission for the undertaking of any medical treatment, including the administration of paracetamol, in relation to my son/daughter, which may prove to be necessary, during his/her time during the week.
I accept photos / videos taken of camp activities, may include my child, and may be used for publicity purposes.
I am happy that contact information supplied on this form will be held securely. (Only for contact purposes, and for a maximum of 3 years after I last attended a Kingfisher event.)
